Oregon Fitness Award
What is the award?
The Fitness (Mayor’s) Award category of the Council’s annual leadership awards program was created to assist Oregon’s Mayors in giving special recognition to individuals in their communities who have made significant contributions in the broad areas of physical fitness. The individual selected by each mayor will receive a personalized “Fitness Certificate” from the Oregon Governor’s Council on Physical Fitness and Sports.

Are there suggested criteria for making nominations?
The individual you nominate may demonstrate some of the following:
- success in the development of a group, community, or statewide program affecting the physical fitness of Oregonians,
- visible promotion of health, fitness, or sports through projects, speeches, and/or publications,
- sponsorship of a community or statewide health, fitness, or sports project influencing the attitudes toward a healthful lifestyle,
- volunteer service to a community or statewide project positively affecting the health and physical fitness of Oregonians,
- scientific or technological contributions in health/fitness-related areas,
- a positive influence in the educational system related to service or curricular efforts toward increasing the physical fitness of students.
Note: Interscholastic athletics coaching success (e.g., wins/losses, league championships, etc.) is usually not sufficient to qualify an individual for an award.
Who may be nominated for the individual Fitness Leadership Award?
Only one local resident of your community may be nominated. A Fitness Leadership Award may be awarded to any volunteer or professional who has contributed significantly to the healthy lifestyles of Oregonians. For example, previous recipients have been teachers, coaches, fitness instructors, premier athletes, business wellness coordinators, sports medicine professionals, board members, etc.
Only 1 (one) individual nomination may be submitted for each city. More than 1 (one) nomination will disqualify all nominations from that city.
What do I need to do?
1) Complete the enclosed nomination form describing the individual you wish to nominate. The form can be downloaded through the links below
2) Provide additional information that will be included in the program for the reception (e.g. a brief description of the individual’s significant achievements; reasons why the individual deserves the award). Remember, this information is intended to be included in the printed program; if you have questions about what to write, please contact Brad Anderson.
3) Nominations should be mailed to the address below. Nominations must be RECEIVED by August 31, 2008 . If you need an extension, please contact Brad Anderson.

Then what?
The Council will review and evaluate all nominations and city applications. Upon completion and selection of awardees, we will notify the individual awardees’ mayors, and present the individual and city awards to the awardees’ mayors at the 2008 League of Oregon Cities Annual Meeting in Salem. A member of the Governor’s Council on Physical Fitness and Sports may be available, upon request, to assist in the presentation of awards to recipients at a local city council meeting.
(Please note that nominations do not automatically qualify individuals for an award. The Mayor’s Award Selection Committee evaluates all nominations, and each year there are nominees that are not selected.)
